Artists in the Classroom
Important Instruction to Participating Artists

Please review and follow the instructions listed below upon receipt of assignment to facilitate planning and a successful visit to your assigned school(s) and class(es).

1. You must have fingerprints on file with the state in order to work inside a classroom.
Search for "Civilian Fingerprints" on the City of San Leandro website.

2. Contact Teacher(s)
Confrim the following with the requesting teacher: date, time, grade level. If there are changes, work it out with the teacher.
If the teacher requests date/time different from your approved schedule, call your ACSL Coordinator for another assignment.
If teacher requests additional time, call your ACSL Coordinator for approval. (You may not get paid for your services if you do not obtain prior approval.)

3. Obtain teacher’s signature at completion of each class
Have teacher sign the Teacher Verification and Request for Payment form before you leave the classroom.
Complete the rest of the form and submit to ACSL by the date indicated on the form. Attach all receipts for supplies. (Supplies should not exceed $2.00 per student. Call your ACSL Coordinator for approval if adjustment is needed.)

4. Complete and return Artist Feedback Report
